Workers’ Compensation
We may use and disclose protected health information as authorized by and to the extent necessary to comply with workers’ compensation laws or similar programs providing benefits for work-related injuries or illnesses.
Uses and Disclosures Where You May Have the Opportunity to Agree or Object
Individuals Involved in Your Care
We may disclose relevant portions of your protected health information to a family member, relative, close personal friend, or another individual you identify who is involved in your medical care or payment for your care.
When you are unavailable or unable to agree or object, such as during an emergency, we may disclose information when we determine that doing so is in your best interest and is otherwise permitted by law.
When appropriate, we will provide you with the opportunity to object to future disclosures.

Request an Amendment
You have the right to request that we amend medical information about you if you believe the information is incomplete or inaccurate.
Requests for amendments should be submitted in writing to Clarity Chiropractic and should identify the information you believe is incomplete or inaccurate and the reason supporting your request.
We may deny your request when permitted by law, including when:
The information was not created by Clarity Chiropractic and the individual or organization that created it remains available to make the amendment;

You have the right to inspect and obtain copies of protected health information maintained about you in our designated record set for as long as Clarity Chiropractic maintains the information.
The designated record set may include your medical records, billing records, and other records used to make decisions regarding your care.
Where permitted by law, we may charge a reasonable fee associated with copying, mailing, labor, supplies, or other costs involved in fulfilling your request.
